2020 Biometric Screening Deadline Extension: Take Your Health into Your Own Hands

The deadline for employees and spouses to earn the Wellness Incentive has been extended to December 31, 2020.

1. Complete a free health screening at one of these locations:
    • Family Health Center (Georgia, Idaho, Indiana, Mt. Sterling, and Oklahoma)
      • Schedule an appointment through the My Premise Health website or app (MyPremiseHealth.com)
    • Onsite at your distribution center (DC) 
      • Where available – ask your manager
    • Physician’s office 
      • A doctor must complete the screening form located on The Compass
2. Meet at least three health goals.
3. Earn up to $600 toward your Health Savings Account (HSA).
